So - this obviously is not because of a carabiner failure - it's because they don't know what they are doing.  If you can - go frame by frame and you will see how the 8-plate is not loaded correctly, there is slack in the system - see where the failure occurs, and how this could be avoided... thoughts?
 

Pretty crazy accident here - a couple good lessons on what not to do... what could have been done from a technique stand point to avoid this?  Watching in slow motion or frame by frame shows it wasn't the carabiner - but rather incompetence...
